I’ve crafted a specific program designed for individuals who are pre-diabetic, or diagnosed with Type 2 Diabetes and strive to manage or prevent progression of this condition through diet and lifestyle.

Though guided counseling I will help you turn this data into personalized nutrition plans and real-life strategies.

Note: CGMs can be purchased over the counter, here, or can be through a prescription from your primary care provider. See below for more details.

Initial Session - Insurance Accepted / $180 if out-of-pocket

  • 60 minute session

  • Detailed nutrition assessment

  • Review of diet, behaviors, medications, labs and supplements

  • Goal setting

  • Personalized nutrition and lifestyle recommendations

  • CGM application and walkthrough

Follow-up Session - Insurance Accepted / $100 if out-of-pocket

  • 45 minute session

  • Continued nutrition education, counseling and recommendations specific to your case

  • CGM Interpretation and analysis

  • Goal accountability

  • Frequency of follow up sessions varies from person to person, however, generally we’ll follow up every 1-2 weeks

Insurance Accepted

Insurance benefits for nutrition counseling are better than ever! More often than not our clients are fully covered with no co-pays, co-insurance, or deductibles. A physician referral is usually required.

I am currently an in-network provider with HMSA, HMAA, UHC, Cigna, Blue Cross Blue Shield, and HMSA Akamai.

​Not all nutrition­ related diagnoses are covered by insurance, and coverage may vary between plans. I will verify your coverage and bill your insurance carrier on your behalf if applicable. I also take FSA, HSA, and cash pay options for those without insurance coverage.

Get Started With Insurance

  • 1) Verify Benefits


    When using insurance, the first step is to ensure your plan covers nutrition counseling. Call the number on the back of your insurance card, starting with 1 (800), to verify your benefits.

  • 2) Obtain a Referral


    Ask your doctor to fax a referral form that includes all the relevant diagnoses you are seeking nutrition counseling for. The form can be faxed to us at 855-610-2036.

  • 3) Book an Appointment


    After we have all the necessary documentation, you can book your first appointment. Intake forms will be sent to you after making your appointment. I use Practice Better to schedule.

Frequently Asked Questions

  • Nutrition therapy is covered by most insurance plans. Coverage varies by insurance plan, often with different rules about referrals, reason for visit, and network/out-of-network providers.

    Before we meet, it is your responsibility to verify your own benefits and understand your financial responsibilities (co-payment, co-insurance, deductibles).

  • At this time the CGM is not automatically included as part of this program. I work closely with your primary provider to prescribe a CGM on your behalf. Depending on your insurance plan your insurance may cover the cost or part of the cost for a CGM. You can also purchase a CGM over the counter at Stelo by Dexcom.

  • The program length is flexible and primarily based on an individuals progression. That said, I recommend up to 3 months for change to take hold.

  • That’s perfectly fine if you do not want to use your insurance for nutrition counseling. I also take cash, check, HSA, FSA, and all major credit cards.
